What is a survey item and how is it different from an assessment item?
In our platform, items can have one of two grading types: assessment and survey. Assessment items have a correct answer and can be scored for correctness, although they may not be depending on the grading policy for the assessment overall. Survey items do not have a correct answer and are always scored for completion.
What does it look like when a student answers a survey item?
When a student answers a survey item it is different from an assessment item in three ways:
- They do not select muddy or clear before submitting their answer.
- They receive full points (if points are assigned) regardless of their answer choice.
- They do not see any "correct" or "incorrect" indicators or feedback after submitting their answer. They simply see a grey checkbox next to that item in the navigation menu, and a prompt to continue.

How do I assign survey items?
- An assessment can contain a mix of survey and assessment items. Survey items will be graded for completion even if the grading policy is for correctness (for example, unlimited attempts to answer correctly or must answer correctly on first attempt).
- A multipart item cannot contain a mix of survey and assessment subparts. The entire item will either be evaluated for correctness (if it is an assessment item) or completion (if it is a survey item).
- One exception: Open-response questions are always graded for completion but can be created as assessment or survey items. The only difference is whether a sample answer appears (for assessment items) or not (for survey items).
- Survey items can be assigned on any assessment type (Readiness, Preclass, Homework, Prep Questions, Practice Test).
- Survey item points can be adjusted like any other item. For example, if you want to add a survey item to the end of a Homework but do not want it to be for points, you can set that item to be worth 0 points.
How do I find survey items in the bank?
- To find survey items in the assessment bank, use the "Grading Type" filter. Please note that for the time being there are no Codon-authored survey items, so you will only find items that were written by yourself or a co-instructor.
- Survey items are indicated in the bank list (left side of the screen) and the selected items list (right side of the screen) by a "survey" label.
How do I create survey items?
1. Click "Create Custom Item" in the assessment builder. This is the same button you click to create a custom assessment item.
2. Select an LO that the item will cover. You may want to create a custom LO for survey items such as "Reflect on your study habits in this course so far and evaluate its impact on your learning in order make plans for the future." or "Provide feedback to make this course and our community more effective and welcoming."
Then click "Create Survey Item".
3. Click the plus button to add a question or feature.
4. When writing questions, you will not indicate a correct answer and there will not be a place to add feedback. Important note: If you create a multipart item, all parts will be displayed and scored as survey type. You cannot mix survey and assessment type questions in a single multipart item.
5. Once you've created a custom survey item, you will not be able to change it to assessment type.
